Complete the new patient intake forms.

After your intake forms are completed, you will receive an email from “nfcnewpatients@nashvillefertility.com” with a link to create your patient portal and complete medical history information. You will receive a separate email from EngagedMD to sign consent forms.

Once the consent forms and medical history information has been completed, our New Patient Coordinator will contact you by phone within 48 hours to schedule a new patient appointment.

At Nashville Fertility Center, we provide comprehensive care by treating both individuals in a couple as patients of our practice. This approach ensures that all aspects of your care are thoroughly addressed. During the initial consultation, both individuals will be seen together so that the physician can make well-informed decisions about the best treatment options. If you are seeking evaluation as a couple, both individuals must complete the required medical history and consent forms before an appointment can be scheduled. Please include your partner in the initial registration. This collaborative process allows us to provide the highest level of care and personalized treatment planning.
